Aims
This topic aims to
- Develop knowledge of the Registered Nurse role in the community to manage complex care needs, implement comprehensive primary health care and health promotion to prevent hospitalisation.
- Develop skills in leading integrated interdisciplinary care teams to support clients in their self-management of chronic/ complex conditions
- Explore the legal considerations, funding models and relevant safety and quality standards relevant to various clients and contexts
Learning outcomes
On completion of this topic you will be expected to be able to:

Determine the ethical, legal, political, and sociological factors that create barriers to care for clients with chronic and complex health needs
2.
Examine primary health care and chronic disease management models and the role of the Registered Nurse within interdisciplinary health care teams to integrate care with a view to prevent avoidable hospitalisations
3.
Critique local and national approaches to health promotion and services available in the community for people with chronic and complex health needs
4.
Examine aspects of health literacy and health education in preventing avoidable hospitalisations
